As a nonprofit community health center network, we discount the cost of care based on your household size and income. Nobody is turned away for inability to pay.
How the program works
The Sliding Fee Discount Program adjusts what you pay based on two things: how many people are in your household, and your household income. The lower your income relative to your household size, the larger your discount, across the services we provide.
The program exists because cost should never be the reason a family skips care. It applies whether you are uninsured, between coverage, or facing costs your plan does not fully cover.
Who qualifies
- Anyone whose household income falls within the program limits, based on federal poverty guidelines
- Uninsured patients, and in some cases patients whose coverage leaves significant costs
- Patients at every one of our six centers, for the services we provide
- New and existing patients alike: you can apply at your first visit

How to apply
Applying is simple and handled with respect, at the front desk of any center:
- Bring a simple proof of household income: a recent pay stub, tax return, or benefits letter
- Tell the front desk you would like to apply for the sliding fee discount
- The application takes minutes, and the discount applies going forward once approved
- No insurance? Ask about Medi-Cal at the same visit: our team helps patients apply, start to finish

Is the sliding fee the same as free care?
Not always, but close in spirit: costs scale down to what your household can genuinely afford, and nobody is turned away for inability to pay. The front desk will tell you your exact discount once your application is approved.
What are the exact income limits and discounts?
They follow federal poverty guidelines and are applied per household at the front desk. Ask at any center and the team will walk you through exactly where your household falls.
Does applying affect my immigration status or get reported anywhere?
The application is about your income and household size, used only to set your discount. If you have concerns about your situation, talk with our front desk; everyone is welcome here.
Can I use the discount and Medi-Cal?
They serve different situations: Medi-Cal is coverage, the sliding fee is a discount when coverage is missing or falls short. Our team helps you sort which fits, and helps you apply for Medi-Cal if you qualify.
