The questions we hear most, answered without fine print.

Do you accept Medi-Cal?

Yes. Medi-Cal is the coverage we serve most, alongside Medicare, IEHP, Anthem Blue Cross, Blue Shield, Aetna, Cigna, Health Net, and UnitedHealthcare. If you are not sure what you have, we check for free.

What if I have no insurance at all?

You are still welcome. Our sliding-fee scale adjusts the cost of care to your household income, and nobody is turned away for inability to pay. And our team helps patients apply for Medi-Cal, start to finish.

What is a sliding-fee scale?

A discount program based on your household size and income: the less you earn, the less a visit costs. You will be asked for simple proof of income, and the discount applies across our services.

How do I find out what my plan covers?

Use the coverage check on this site or call 877-335-2559. Tell us your plan, and our team confirms what is covered before your visit, free and with no obligation.

Becoming a patient

Are you accepting new patients?

Yes, at all six centers. Request an appointment on this site, call 877-335-2559 Monday to Friday 9 to 5, or start with the chat any time.

What should I bring to my first visit?

A photo ID, your insurance card if you have one, a list of any medications you take, and, if you are applying for the sliding-fee discount, simple proof of income. If you are missing something, come anyway and we will figure it out together.

Can my whole family be seen here?

Yes, that is the point of a community health center: pediatrics for the kids (through age 21), family practice and women’s health for adults, and care for seniors and veterans, often at the same location.

Do I need a referral?

No referral is needed to become a patient or book a primary care, pediatric, or behavioral health visit. When you need specialty care beyond our walls, our team arranges the referral for you.

Locations & services

Where are you located?

Six centers across the Inland Empire: Chino, Montclair, Upland, Rancho Cucamonga, Hesperia, and Palm Desert. Every location page has the address, phone, and directions.

Do all centers offer the same services?

Core services are shared across the network, and some services vary by center. Tell us what you need when you book and we will match you to the closest center that provides it.

Do you offer mental health care?

Yes: counseling, psychology, psychiatry, and family counseling are core services, for adults and children. You can start through the mental health door on this site, no referral needed.

Do you offer dental care?

We do not provide dental services on-site. Our team can point you toward dental resources in the community, and if you have Medi-Cal, we can help you understand its separate dental program.

What is the mobile clinic?

A clinic on wheels that brings care into neighborhoods and community events around the Inland Empire. Use the mobile clinic form on this site and we will let you know when it is near you.

Visits, records & the portal

What are your hours?

Generally Monday to Friday, 9 AM to 5 PM, and some centers vary slightly. Your location page and its front desk have the exact hours. The chat on this site is available any time.

What if I need help after hours?

If it is an emergency, call 911. For a mental health crisis, call or text 988 any time. For anything that can wait until morning, send a message through the chat or patient portal and we will pick it up the next business day.

How do I see my records, results, or messages?

Through the secure patient portal (the Patient Portal link in the footer). If you need help signing in, call your center and the front desk will walk you through it.

Do you have Spanish-speaking staff?

This website is fully available in Spanish, and we serve a large Spanish-speaking community. Tell us your preferred language when you book and we will make sure your visit works for you.

No insurance, or not sure what you have? You are still welcome, and our team helps you apply for Medi-Cal.
